To make your free consultation as meaningful as possible, you should have the following readily available:
- A copy of any individual disability insurance policy or plan that you purchased.
- A copy of any group insurance policy or plan that you purchased or are enrolled in because it may contain a premium waiver that provides for continued coverage for free while disabled. Your employer is obligated to provide you with the relevant documents.
- A list of your physicians, hospitals and other treating or consulting medical sources, with their records. Make sure that you have an address and telephone number for each medical source.
- A list of your prescribed medications and devices. Your pharmacy should be able to give you a computerized print out of your records for the last year or two.
- Your work history for the last 15 years. If you have a resume, it should help you summarize where you you worked and what kind of work you performed.
- Your birth certificate.
- Records of your earnings. The Social Security Administration regularly mails out Social Security Statements, which list your entire earnings record. If you do not have a copy of your Social Security Statement, then recent tax records would be helpful.
Social Security disability cases are typically handled on a contingency basis that is limited to 25% of any retroactive benefits.
