For all other disability:
- A copy of any individual disability insurance policy or plan that you purchased.

During a free consultation with a lawyer, we will explain what forms to submit and how to avoid potential mistakes in completing them. If retained, we will help prepare and submit the necessary forms and evidence in order to try to maximize your chances of obtaining disability benefits as quickly as possible.
Social Security disability cases are typically handled on a contingency basis that is limited to 25% of any retroactive benefits.
